Minors (Under 18)
 

For any piercing or service a parent or legal guardian must be present for the entire appointment. The minor must provide a photo ID and a valid, non expired, government issued document that shows their date of birth. The parent/legal guardian must provide a valid, non expired, government issued photo ID. The parent’s ID and minor’s ID must have matching last names and addresses. If the last name and/or address doesn’t match, the minor’s original birth certificate must also be provided. If the name on the parent’s ID has changed since the birth certificate was issued, a marriage license or change of name form will also be required. If you are the legal guardian of the minor you will need to provide the original permanent guardianship papers. 
 

Acceptable forms of ID for parents/guardians include

• Driver’s License

• Federal or State Issued Photo ID

• Passport

• Military ID
   

Acceptable forms of ID for minors include

• Driver’s License or State Issued Photo ID with the last name and address matching the parent’s ID

• Driver’s License, State Issued Photo ID, Military ID, Passport, or School/Work Photo ID and Birth Certificate
